Preheat oven to 350°F (175°C). Grease a loaf pan with butter.
In a large bowl, mash the 3 ripe bananas until smooth. Stir in the blended cottage cheese, maple syrup, eggs, melted butter, salt, cinnamon, and vanilla until well combined.
Add the flour and baking soda, mixing until just incorporated. Fold in the chocolate chunks.
Pour the batter into the buttered loaf pan. Slice the extra banana in half lengthwise and place it on top of the batter. Brush with melted butter.
Bake for 60-65 minutes, or until a toothpick inserted in the center comes out clean.
